This Page is for anyone who wants to try out our practice (or perhaps you have been away for a few years and need to come back in).

May We Suggest:
1. Decide why you want to see a medical professional. You may want to include problems and hoped for solutions.
2. If more than one issue, write down the list! (We may not get to all problems today, but we want to know!)
3. Write down a short list of your medical history and your medications (or print and fill out the site History Form)
4. Read the Biographies on this site and see if a particular provider seems to meet your needs more than the others.
5. Call for an appointment. 402-502-3600. The operator may also be able to answer some basic questions about the providers.
6. Look at the Forms Page on this site. At a minimum, please print and fill out the New Patient and Medical History forms (the others forms are optional). Also records sent to us can take a month to arrive, so plan ahead!
